Pre-Participation Requirements

Before the first try-out of every season, each student-athlete in the CMIT Lacrosse program is required to (1) Create a Form RELeaf account and (2) upload a pre-participation physical to the Form RELeaf account. Both requirements must be met before the start of the first try-out to be considered eligible for interscholastic sports.

Pre-Preparticipation Requirments :

1. Form RELeaf: ​​In order to upload your pre-participation physical, you will need to create a Form RELeaf account that will serve as parental consent to participation upon submission. For further instructions in creating a Form RELeaf follow the instructions linked here.

2. Pre-Participation Physical: All student-athletes must have a physical conducted by a physician or a nurse practitioner to participate in interscholastic sports. Student-athletes in the CMIT Lacrosse program should make (3) copies of their physical for their own files, the coach's medical kit, and for the physical trainer in addition to uploading a copy to Form RELeaf. 3. COVID-19 Return To Athletics: All student-athletes must fill out the following COVID-19 disclaimer prior to participation in athletics. The disclaimer form that must be filled out can be found linked here.
